Patient Information
First Visit
We are excited to see you for your first visit. Whether your first appointment is for a cleaning and examination, consultation, or is problem focused, we use this opportunity to get to know your needs and address any questions or concerns you may have. We strive to get to know you and understand your past dental experiences as well as your goals for your smile.
We ask that you to arrive for your appointments on time, as we only deviate from our schedule in the event of dental emergencies. If you are unable to keep your scheduled appointment, please give us at least 48 hours notice. A $25 missed appointment fee may apply for repeat cancelations without notice or repeat failed appointments

Payment Options We accept cash, checks, debit cards and most major credit cards. We also offer CareCredit® for patients who qualify. Care Credit allows for monthly payments for your dental treatment often with no interest. Click here to apply for Care Credit

Dental Insurance PolicyWe participate with most major dental plans and are in-network for most plans. Even if we are not currently preferred providers in your network, we will be happy to help you understand your benefits to maximize them.
Please bring your insurance card or information with you to your first visit. We submit and track all claims for you and will ask you to confirm this information at subsequent visits so we continue to process them correctly. Please remember that you are responsible for the payment of all fees, regardless of whether or not insurance pays a portion. We are happy to submit pre-determinations to help you understand what insurance may pay on your behalf. |
